What Is Carpal Tunnel Syndrome?

In the simplest terms, carpal tunnel syndrome occurs when nerves in the wrist are compressed, leading to pain or a lack of sensation in the hand and fingers.

The carpal tunnel is a passage in the wrist surrounded by ligaments and small bones. An important nerve, the median nerve, passes through this tunnel and connects to the thumb, index finger, middle finger, and ring finger. Sometimes, the carpal tunnel becomes narrow or swollen, which compresses the median nerve, causing decreased blood flow and poor electrical signals into the hand and finger. 

Symptoms can include numbness, tingling, weakness, burning, and pain, sometimes radiating up into the wrist or forearm. This can occur in one or both hands.

These effects can be debilitating and permanent if not addressed early in the disease process.

Carpal Tunnel Surgery

Dr. Nathan Lesley explains the benefits of carpal tunnel surgery using modern techniques. Recovery is much faster now than in years past, thanks to less invasive techniques. In fact, patients can have full use of their hand immediately after surgery, although heavy lifting and other strain must be avoided for up to two weeks.

How Does Carpal Tunnel Surgery Work? A Procedure Timeline

Surgery to treat carpal tunnel syndrome is known as "carpal tunnel release." At The Hand to Shoulder Center in Fort Worth, TX, we perform both open and endoscopic carpal tunnel release. Regardless of which method you choose, the procedure follows the same basic timeline:

Anesthesia

Carpal tunnel release surgery can often be performed with only local anesthesia. Some patients elect for general anesthesia or IV sedation. Those who choose more powerful anesthesia need to arrange for someone to drive them to and from surgery.

Incision

In an open surgery, your surgeon will make an incision in the palm to expose the inside of the hand and wrist. In endoscopic surgery, you will have one or two smaller incisions where the surgeon will insert a surgical camera (an endoscope). This allows the doctor to view the inside of the wrist without exposing it.

Ligament Division

Operating from either the inside or the outside of the wrist, your surgeon will divide one of the ligaments that make up the barrier of the carpal tunnel. This releases pressure on the median nerve. Tissue around the nerve might also be removed.

Stitches

Finally, your surgeon concludes the procedure by stitching the incisions closed with a focus on minimizing discomfort and scarring.

Recovery from Carpal Tunnel Release Surgery

After your carpal tunnel release, you will receive specific instructions on what you should do to reduce swelling and discomfort in your hands and wrists. Depending on the state of your median nerve before the procedure, it can take several months for full strength to return. That said, nighttime symptoms can sometimes improve the night after surgery!

To experience the best possible recovery, you should wear the wrist brace provided and avoid any strenuous activity. In some cases, we may recommend physical therapy exercises for your hand and wrist to encourage the healthy recovery of your muscles and nerves. Repetitive motions such as typing, waiting tables, or playing an instrument can cause the tissue in the wrist to swell and put pressure on your nerves.
